Most of Ohto's shapeshifting remains discreet beyond the bone growth, like stopping hemorrhage or shifting his heart to the side. It's a lot of minor stuff that makes killing him extremely hard, and his body is already perfect for fighting. An extra limb is cool on paper, but it makes him need to relearn how to fight with a third one, and it's not free. He doesn't make biomass out of thin air; it's either from his body or mana pool.
