The doctor looked at Gun Woo who was clearly had no idea or thinks he already knows what was happening, fooling himself unconsciously. He then looked at Kwan who had his eyes widened a little bit. It was the reaction that he was supposed to get from Gun Woo but it looks like only Kwan understood the message completely. The two were looking at each and having a silent conversation where the doctor was silently apologizing for Gun Woo who was a bit lacking on that part. Kwan had completely understood and he had thought that he should be the one to say it instead as he cleared his throat.
