Kakashi gave Kayn a strange look and said, "Don't worry, I don't have anything with me except a book."
Kayn smiled without saying anything. He simply stood there, unmoving.
Changing clothes in the current situation can avoid a lot of trouble when moving forward with the plan.
It is better to be safe than sorry.

They didn't know what made it crucial but as Kayn said before, during the enactment of the plan, he's the commander. At that time, they should simply cooperate with him, without asking questions.
Anyway, right after this, Kayn followed inside the house and entered the 'operating room'.
"... Doc, can you come over here please, I have some questions."
The doctor who was in the middle of putting on a rubber glove came over.

Hearing that, Kakashi narrowed his eyes and said calmly "A ninja is someone who endures everything that ordinary people cannot endure. When Rin... someone performed the transplant on me, they didn't use anesthesia, but I survived."
"So just do it with confidence. I can bear it."
Regardless of what Kakashi said, the doctor took out a syringe and drew the liquid from the unlabeled bottle.

"I said, I don't want anesthesia."
