He recognized some of them from his time back at Winterfell; they had always been loyal to him and now they were here serving under the Grandmafias.

Damien gritted his teeth together tightly as he realized that this was going to be harder than he thought. He couldn't take out all these lower mafias without starting an all-out war with their superiors.

But first things first – he needed to save Elena before it was too late.

He pulled out his gun from his holster and carefully made his way through the corridors of the hospital until he reached the room where he knew they were keeping Elena captive.

Without hesitation, he kicked open the door only to be met by fake Elena holding a syringe filled with some unknown substance pointed at an unconscious figure on a bed - it was Elena.

"Let her go," Damien growled, his finger tightening on the trigger of his gun.
