There's nothing wrong with checking your notes every once in a while. It's good that you're talking consistency so seriously. Thanks for the chapter

It's not as simple as it being possible to get along with someone quickly, the mc has emotional detachment issues and is actively trying to distance himself from other people. In order to "click" with someone you must first be in a state of mind where "clicking" is possible/plausible and mc is explicitly not in such a state nearly 24/7 as a defining part of his character.

It's super easy for a writer to add randomness to a story, just flip a coin or roll dice for decisions.

It's possible that Orochimaru had extra samples of Kabuto's blood stored for situations like this, he's one of the most cautious in regards to avoiding death after all.

While it's not everyone there's still plenty of people in the ninja world outside of clans and nobility who have last names, Sakura Haruno, Rock Lee, and Might Guy to name a few.

Sure, so as we've established there are some sounds that she can't make at all like "p" and "b" which have to be left out either way, but there's also sounds that when she tries to make them will produce a different sound instead like "f". The method you had been using was to omit some and mispronounce others, my alternative is to omit everything that doesn't sound right too. To demonstrate I'll alter the simple sentence "No problem, this is fine" with each method. Yours: "No 'ro'len, this is thine", Mine: "No 'ro'le', this is 'ine" The upside of doing it this way is that it's technically easier on you to only learn which sounds can't be made than to also learn which are mispronounced and how, though as you might have noticed it can get a bit difficult to read when so much of a word or sentence is missing. I realized while writing this that I should warn you about words pronounced differently than their spelling like "subtle" and words with multiple omissions next to each other like "suppression" since it could be mistaken for quotation marks once changed. Anyway, I just wanted to give you some ideas for how you could change it if you wanted to, I'd personally recommend the vocabulary change instead of this method since its simpler to adjust in the long run and easy on everyone. It's up to you though

It's not a totally bad idea, another alternative is to have her completely omit sounds she can't make which would work wonderfully with the apostrophe thing. The story idea is interesting and I'm looking forward to what you make next.
