Based on nirvana mutation the higher up you go I believe he is, and just mutated. And instead of losing himself and becoming animal like or whatever else is possible. He made it through at the end, but mutation is mutation so his looks changed, and he lost some memories while retaining others
Remember the more nirvanas the more chance for mutation, being his half(vertically) metal I would say he(his human side) looks different because of that too. For all we know before he lost himself(appearance and memories) he was able to make it somehow and that’s why he has some memories but lacking others
