I’m pretty sure that the cursed spirit emits negative magical energy so even if it constantly heals a person it also slowly kills them anyways since it’s like poison in your veins.
Asterion was the same; although he could create a cursed spirit capable of healing him, he wouldn't dare to put something like that inside his body. The negative magical energy would definitely corrode his body from the inside out.

He’s not really cruel to everyone, he’s indifferent to most people and actually quite nice to those he can help the only thing I would label “cruel” about him is his indifference towards the people who his family hurt, it’s like even if someone in his family destroyed a whole town he wouldn’t care but if he sees someone or something else try to destroy a town he would kill them and we can’t really say he’s only loyal to his family either because we haven’t seen him form a friend group or the like so we wouldn’t know if he’s only loyal to his family only, and he’s pretty hardworking to if you think about it even before joining the blacks he still trained until he was almost semi special grade for 3 1/2 years, and for the gryffindor he’s talked about being scared and feeling like he’s gonna die if he stops training and to still keep going in spite of that is a testament of his will, and also you can’t tell me he isn’t worthy of being a gryffindor by the sheer audacity of him calling grindelwald out about him not having kids and him saying something about dumbledore(”your old lover”) in the masquerade party.
