He kept groaning in pain, and when he couldn't hold it in any longer, he would scream as if forgetting that he wasn't in his palace where he was free to do whatever he wanted.

He was currently still in Wu Jinhai's palace, and people outside the pavilion he was staying in could probably hear his screams and think something was wrong with him.
