As long as one is a member of the Nara, Yamanaka, or Akimichi clan, they must accept the curse seal under the supervision of the successive Hokage when they reach the age of four. In simple terms, once the curse seal takes effect, if the Hokage wants you to live, you live; if the Hokage wants you to die, you die, without any room for resistance.
