I have seen one in a d&d game I played once it was a shirt that when touched by a living being directly or indirectly within a 15 foot radius would dissolve into a 20 foot radius stinky gas while also teleporting you back to the last place you slept/unconsciousness and teleporting it self randomly in the world within a 30 foot radius of a living being before reforming into another random Item/material with the same effects so it dose nothing can't be worn or picked up and you never know where it will be or what it will be or what living being might come into direct or indirect contact with it and be teleported to the last location they slept/unconsciousness at while also making them smell awful.
No such thing as a bad item.

Water release just sort of exploded into billions of needles moving like bullets in every direction from the epicenter. In simple terms it was perfect for killing groups of enemies, not weak but pretty lackluster. The earth release version was terrifying and I directly labeled it a forbidden technique the moment I saw what it did. Upon impact it detonated into a bright dome in which EVERYTHING inside it was immediately fossilized. Plants, animals, people it didn't discriminate and turned everything it touched to stone in that whole area. Destruction I was fine with I could fix destruction but there was no repairing the effects of this technique.-
