I know the author focuses on carefully laid intricate plot details, world building, strategy and politics more in this story but this chapter would have been the best place to pull some next level Uchiha type twist if the Trinarian representative had declared something like they had broken the mana oath and tried robbing some Imperium ships resulting to the Imperium silently attacking them and gaining control of their empire within just a few days before the rest of the Conclave had even been informed. Then the entire war would have been shown as a flashback. But again, I understand this isn't the author's style and honestly, it would have left quite a few plotholes like how did they get access to the warmhole technology for the attack to have succeeded and all. But it would have definitely been one big good twist.

It probably happened in the episode when Ben had drunk some elixir of youth. It was a magical liquid that reversed his age. It might have happened some other time too but ever time it was because of some sort of mystical attack or magical spell. As for the cold thing, it was canonically confirmed it was an omnitrix glitch due to a bug in the prototype. That and the fact why most of his aliens continued wearing his original white and black shirt even after he had transformed. There were lot of bugs in the original classic prototype. All of those were corrected in the later versions of the omnitrix and the ultimatrix.

Not really. The omnitrix makes the user the prime of any species they transform into. it means you are becoming the captain america of the species that you select. That was the reason why Ben was able to beat Looma Wind, the princess of the "Four Arms" species even though females of the race are said to be stronger than males. Its also the reason why Ben always transforms into the adult of every alien race he transforms into.
