Ken being exhausted is also because of his MEDP being overused. As far as I know, even Tohno and Shiki could still got the backlash from it. It's not just his stamina and physical body problem. But feel free to correct me if I'm wrong.

Understandable, my knowledge of Fate's magic is definitely not deep enough but even I understand that some servants could compete with those cosmic beings or even overpowering them. I'd say servants like King Hassan still have a 50-50 chance againts the real phoenix, not counting their weak avatar like jean or scott.
