Li Lu’s mother is Mrs. Hu. Also Lai Manting is supposed to be a legitimate daughter of the main line but isn’t named Leng or Feng after her mother. Honestly the only thing that makes sense to me is that Li is a misspelling of Lai and everyone except the main legitimate line uses Lai. And before Madam Feng became the main wife her kids weren’t legitimate and therefore used the Lai name. But that’s just a guess. I don’t know if it even makes sense.
Thinking it over, the only possibility was that either Lai Manting or Li Lu had spread the word.

Is the author really trying to treat children’s sickness like it’s a competition?
[Speaking of frailty, Yan Wang is the genuinely frail and pitiable one, right? I have a best friend working at Compassion Hospital in City C, and she kept telling me about a child in the ICU who has had to live in the hospital since he was born, spending hundreds of thousands every month just to stay alive, so beautiful yet so pitiable! I sent her Wangwang's video last night, and she said that's the kid from their hospital!]

Except she did say it all the time and she also abused them. Of course they’re scared and distrustful. So yeah I’m sticking with entirely her fault.
It was partly the fault of the female supporting character that they were so easily startled.
