Sasuke really hadn't known this, so upon hearing the news, he was utterly stunned.
The Mangekyou Sharingan—a coveted, ultimate evolution of the Uchiha clan's dojutsu that every member aspired to achieve.
Yet this ultimate form had such a significant drawback? The more it was used, the closer it brought the user to blindness?
For a clan that relied on their eyes, this was nothing less than a dreadful curse.
"Akira, could it be..." Sasuke looked at Akira, his face showing shock and confusion.
"Itachi has had his Mangekyou for years, and he's used it a lot. He's already on the verge of going blind," Akira replied, choosing not to dwell on himself, instead focusing on explaining Itachi's situation.
"I see... that makes sense now," Sasuke replied, his expression darkening as realization dawned. "That's why during our fight, even though my abilities were clearly inferior to his, I felt like he wasn't at his best. So this was the reason."
"The main reason is probably because Itachi was holding back," Akira thought to himself, casting Sasuke a sideways glance.
Sasuke, it seemed, had not once considered that Itachi had been pulling his punches. Itachi truly was a masterful actor and deceiver—even in death, he had Sasuke completely fooled.
But since Itachi had deceived Sasuke thoroughly, Akira chose not to reveal the truth.
Instead, he continued, "There is a way, actually, in Uchiha history, to overcome the Mangekyou's drawbacks."
"What is it?" Sasuke asked, his curiosity piqued.
"You need to transplant the Mangekyou of a close sibling," Akira explained.
"Sasuke, you're in luck. Not only did you awaken your own Mangekyou, but Itachi also had one."
"So, in theory, if you transplant Itachi's Mangekyou, you'll gain both sets of ocular power."
"This will not only enhance your abilities, but also grant you eternal clarity of vision. This is what we call the Eternal Mangekyou Sharingan."
"Transplanting a sibling's Mangekyou Sharingan to gain double the ocular power and achieve the Eternal Mangekyou?" Sasuke repeated, looking surprised.
If this was true, why had Itachi never tried to take his eyes when he was on the verge of blindness?
"Alright, we have Tsunade with us, so tomorrow, let's arrange for your Mangekyou transplant," Akira said after giving Sasuke an overview of the situation.
"Got it," Sasuke nodded, not rejecting the idea.
The next morning, after everyone had woken up and washed up, Akira led them to a hospital in a nearby town.
By now, Akira and his team were within the borders of the Land of Fire.
When they heard that Akira's team, with the legendary Tsunade herself, was requesting the use of their hospital facilities, the staff at the hospital considered it an incredible honor.
Soon, Akira took Itachi's body out from the sealing scroll and handed it over to Tsunade.
In truth, transplanting the Sharingan was a very straightforward procedure.
Think back to how Kakashi's Sharingan was transplanted.
It was done casually while on a mission, with relative ease.
And during the Fourth Great Ninja War, Sharingan and even Rinnegan transplants were done without any surgery at all.
This has been humorously referred to by fans as the Uchiha's eyes being "plug-and-play."
Even Obito, after death, was able to transfer his eyes to Kakashi in the spirit world—that was even more over-the-top.
So, it seemed that transplanting a Sharingan was even easier than a regular eye transplant.
With Tsunade herself overseeing the procedure, the transplant was completed effortlessly.
After resting for half a day, Sasuke had fully adjusted to his new eyes.
He opened his eyes, and they transformed into the Mangekyou state. The pattern in his eyes now was a fusion of his original Mangekyou design and Itachi's.
Upon opening the Eternal Mangekyou, Sasuke felt a tremendous surge of power—it was as if his abilities had doubled. To put it in terms of strength values for a Kage-level shinobi:
Previously, Sasuke's overall strength was around 10,000.
Now, after the transplant, his power had reached about 30,000.
The magnitude of the increase was something Sasuke could clearly feel within himself.
Of course, Sasuke's knowledge and experience now far surpassed his original state in the storyline.
Thus, even though his new strength was thrilling, it didn't make him overconfident.
In the original story, when Sasuke transplanted his Mangekyou Sharingan, he became incredibly arrogant.
He thought himself invincible—first deciding he'd deal with Killer Bee, only to be utterly beaten.
Then, he stubbornly crashed the Five Kage Summit, getting beaten by the Raikage and nearly dying from the Third Tsuchikage's Dust Release.
But now, surrounded by a team of Kage-level fighters, especially with someone like Akira, a super-Kage-level force, Sasuke understood the vastness of the world.
So, even with his new Eternal Mangekyou form and a power boost, he was acutely aware that there were still plenty of people far stronger than him, and he dared not become arrogant.
With the transplant complete, Akira's team continued their journey and finally returned to the Leaf Village.
However, upon arrival, Kakashi and the others looked stunned.
Why? Because the Leaf Village appeared to be in a state of reconstruction, as if it had just endured a massive battle.
"What happened here? Did someone attack the village? Was it Urashiki Otsutsuki?" Jiraiya asked, visibly shocked.
"Or could it be that another village attacked?" Hanabi murmured, confused.
"How strange. We went to attack the Akatsuki, but who could have dared to attack the Leaf in the meantime?" Even Naruto was baffled as he spoke.
"Let's go. Let's head inside," Akira said, showing little surprise as he led everyone into the village.
Akira had already anticipated that Toneri Otsutsuki would come for Hinata, which was why he had left Sasuke in the Leaf.
The destruction they were seeing now was clearly the aftermath of Sasuke and Toneri's battle.
"Akira, you don't seem surprised at all," Sasuke remarked, turning to Akira with a puzzled expression.
In response, Akira simply gave a mysterious smile, telling everyone that their mission was complete, and he needed to make a brief report at the Hokage Tower.
After leaving those words, Akira gently patted Hanabi on the head, signaling her to go check on her sister.
Then, using Body Flicker, Akira vanished from sight.
With their return to the village, everyone else dispersed, intending to learn more about what had happened to the Leaf.
It had been strange recently in the Leaf Village.
Why?
On one hand, ever since the Uchiha clan took charge, the Leaf's overall power had grown tremendously.
But on the other hand, the stronger the Leaf became, the more frequent these attacks seemed to be.
Since Akira's return as a full-fledged dragon, it had only been half a year.
First, the Akatsuki attacked, which led to the awakening of both his and Sasuke's Mangekyou Sharingan.
Then, there was the attack by the Akatsuki along with Urashiki, and now Toneri's assault.
In just this short half-year, the Leaf had been attacked three times.
Who could believe that?
After silently reflecting on this, Akira made his way directly to the Hokage Tower.
"You're back?" Fugaku greeted Akira upon seeing him.
"Yes, I'm back, Lord Hokage," Akira replied seriously, nodding.
Fugaku opened his mouth, seemingly wanting to ask about the mission—whether it was successful—but the words wouldn't come out.
After all, this mission involved the life and death of his own son.
"The mission is complete. Itachi's Mangekyou has been transplanted to Sasuke."
"Clan Leader, I'll hand over Itachi's body when we reach the Uchiha district," Akira added, understanding Fugaku's feelings.
Upon hearing this, Fugaku swayed slightly, feeling dizzy.
Still, he held himself together and gave a silent nod.
Akira knew that Fugaku needed some time alone, so without saying more, he turned and left the Hokage Tower.
As for asking about Toneri's attack, Akira knew this wasn't the right time to ask Fugaku, so he decided to go find Sasuke instead.
Getting the details from him would suffice.

In the original story, Hiruzen Sarutobi, the Third Hokage, had also experienced a decline in strength as he grew older, and it was evident that Akira's grandfather was no different.
After a long walk and some heartfelt conversation, Akira returned to the clan compound with his grandfather once night had fallen.
Once home, Akira sought out Sasuke to ask him about Toneri's attack.
In fact, the moment Toneri Otsutsuki attacked Konoha, Sasuke had immediately sensed his presence.
After all, Sasuke's primary purpose in staying in this timeline was to guard against an attack by Urashiki Otsutsuki.
Thus, Sasuke had raised Konoha's defense to a level meant to counter super-kage-level threats.

"This was his choice. Itachi sought his own path—he wanted redemption, but given the crimes he committed, there was no way for him to return to the Uchiha clan," Akira said.
These words made Sasuke nod silently. He understood this sentiment deeply.
In his own timeline, even if he wanted to protect Konoha, he could only do so from the shadows.
After betraying the village, there was no realistic chance for him to return to Konoha either.
Leaving Akira and Sasuke's conversation aside, elsewhere that night, Sasuke lay in bed, restless and unable to sleep.
In fact, these past days, Sasuke had barely been able to sleep at all.
He had killed Itachi with his own hands, and Itachi's eyes had been transplanted into him—something that left Sasuke feeling deeply conflicted.
If Itachi had truly been so sinful, why hadn't he, with his near-blind Mangekyou Sharingan, tried to take Sasuke's eyes to gain the Eternal Mangekyou?
Yet, Itachi had never had such intentions. Why, then, did he stay behind to cover the Akatsuki's retreat, even at the cost of his own life?
Vaguely, Sasuke felt that things weren't as simple as they seemed. There had to be more that he wasn't seeing.
As Sasuke lay tossing and turning late into the night, lost in his chaotic thoughts, his eyes suddenly opened on their own.
The Mangekyou Sharingan's pattern appeared in his eyes, and Sasuke realized that he was seeing an entirely different vision. This was Itachi's transcription seal, something Itachi had left in Sasuke's eyes long ago.
Itachi had sealed certain memories within his eyes, and the condition for triggering this seal was Sasuke's return to Konoha, specifically the first night he slept in the village.
As the transcription seal activated, Sasuke was pulled into a genjutsu. In this state, Sasuke found himself witnessing numerous scenes from Itachi's perspective.
First was the Third Shinobi World War. Sasuke watched as a dying enemy shinobi was spared by Itachi. However, once the shinobi realized Itachi was from Konoha, he showed no gratitude and attacked, forcing Itachi to slit his throat.
This was the first time Itachi began to question the nature of life, war, and peace.
Next, Sasuke saw Itachi mourning the death of his comrades. He watched as a young Sasuke, barely crawling, made his way into Itachi's arms, noticing tears dripping onto his body.
Looking up, a young Sasuke, in his childlike voice, asked Itachi what was wrong.
Itachi's eyes, however, had already turned red—his Sharingan had activated.
Then came Itachi's days at the Academy. Finding the curriculum too easy, Itachi sent a shadow clone to attend classes while he trained with Shisui at the training grounds.
It was clear that Shisui mentored Itachi like an elder brother, and the bond they shared was profound.
Through this genjutsu, Sasuke witnessed Itachi's growth firsthand—a journey befitting the title of genius.
Within the Uchiha clan, Itachi and Shisui were regarded as the twin prodigies, the pride of the Uchiha.
Through Itachi's perspective, Sasuke saw the clan's arrogance. Assigned to the Konoha Police Force, the Uchiha's duties naturally put them at odds with the village's civilians, and the tension between them grew worse.
The root of the issue was the Nine-Tails' attack. Many had seen the beast's eyes take on the Sharingan's pattern, unmistakable evidence that it had been under the control of the Sharingan's power.
This led the villagers of Konoha to suspect the Uchiha as the ones behind the attack.
As these memories surfaced, Sasuke's own childhood recollections began to become clearer.
Indeed, long ago, before the Uchiha rose to power, his fellow clan members had not been on good terms with the villagers.
The Uchiha were arrogant, considering themselves superior to everyone else.
At the time, Sasuke was only six or seven years old, so many of these events had been vague and abstract to him.
Now, revisiting those moments through Itachi's memories, the fifteen-year-old Sasuke saw things clearly—the relationship between the Uchiha and the rest of Konoha was indeed fragile and volatile.

Similarly, the village elders wanted Itachi to keep an eye on the Uchiha's movements.
Caught between the village and his clan, Itachi endured unimaginable torment.
Throughout these visions, Sasuke noticed Itachi gradually becoming aware of Akira's existence.
Akira's talent had naturally attracted Itachi's attention.
Initially, it was because Akira was Shisui's disciple.
But over the course of just one year, Akira went from unlocking the Sharingan after a fall to passing the graduation exam on his very first day at the Academy. This caught Itachi's eye, and he began to pay more and more attention to Akira.
Amid the clan's growing calls for a coup, Itachi and Shisui often discussed how best to prevent an internal conflict between the village and the Uchiha.
And then, things finally took a turn.
One day, while Itachi was home, he heard Akira calling for help. The message was simple: Shisui was in danger.
Upon hearing that Shisui was in trouble, Itachi immediately followed Akira.
They quickly found Shisui, who was indeed in a fight against members of Danzo's Root.
More importantly, one of Shisui's Mangekyou Sharingan had been taken by Danzo.
Through the genjutsu, Sasuke was now able to witness these events from Itachi's perspective for the first time.
