One interesting plot twist could be that Percy discovers his demigod powers are somehow linked to ninja abilities. For example, his ability to control water could be enhanced in a way that is similar to how ninjas use stealth in water. Another twist could be that the ninja world he enters is actually a hidden part of the demigod realm, and he has to team up with both demigods and ninjas to fight a common enemy, like an ancient evil that threatens both worlds. Maybe he also finds out that some of the ninjas are descendants of minor gods, which adds a whole new layer to the story.
A really cool plot twist could be that the gods send Percy to the ninja world as a sort of punishment or test. But while there, he discovers that there is a prophecy in the ninja world about a 'foreign hero' (him) who will save them from a great disaster. This forces Percy to take on a new role and identity in this strange new world. He has to learn their ways quickly and make allies among the ninjas, all while trying to figure out how to get back to his own world and fulfill his duties as a demigod. And there could be a character in the ninja world who looks exactly like one of his friends from Camp Half - Blood, but is actually an enemy in disguise.
