One common theme is the struggle for survival. In the face of a smallpox epidemic, characters have to fight not only the disease but also the resulting chaos such as food shortages and social unrest. For example, in books where a town is hit by smallpox, people are forced to find ways to keep themselves and their families alive.

Another theme is the medical aspect. This could include the search for a cure or treatment. In some smallpox fiction, there might be a heroic doctor or healer trying different herbal remedies or experimental treatments. It shows the desperation and the hope that comes with trying to combat the disease. Also, the theme of isolation is often present. Those with smallpox or suspected of having it are often quarantined, and this isolation can lead to loneliness, fear, and a sense of being cut off from the world, which is a powerful theme in such fiction.
